Self Pay
The cost is $150 per visit. This cost covers the care that we give to you and your baby during that visit.
We can provide you with a “Superbill,” or itemized receipt that you can submit as a claim through your insurance provider if you would like to attempt reimbursement on your own.
Third Party billing
The Lactation Network and Wildflower Health are middle men between us (Latched Louisville and you) and health insurance companies. With this arrangement, these companies assume all risk of denied claims, underpayments and wait times. They also have contracts with commercial insurance plans to cover your visit fully without any cost sharing (meaning if you are approved, you will not have any patient responsibility for cost such as a copay or deductible).

You must get approval prior to your visit. It takes a few days to get approval, so you must mark your visit as “urgent” to be seen quickly.

$1 booking fee for wildflower and Tln patients
This is for every appointment scheduled.
If there is a cancellation, missed appointment, or reschedule fee, your payment card on file will automatically be charged as described in the Appointment Modification Policy below.
Appointment Modification Policy:
For cancellations and reschedules with 48-24 hours notice:
There is a $50 fee.
This can go towards your next appointment.
Self Pay:
You will be refunded what you have paid for the appointment, minus a $50 fee.
TLN/Wildflower:
Your payment card on file will be charged this $50.
For cancellations and reschedules with less than 24 hours notice, as well as missed appointments:
There is a $50 fee.
This does not go toward your next appointment.
Self Pay:

Why do I need to have a credit card on file to book a visit?
If there is a cancellation, missed appointment, or reschedule fee (as described in the Appointment Modification Policy), the payment card on file will be automatically charged.
Why no longer take insurance?
This difficult decision has come after countless challenges with specific insurance plans. We have had many plans either not pay enough to sustain Latched Louisville, or not pay at all after a visit. While we have loved those visits, we do need to be paid to stay open so that we can help families in their lactation journeys.
What is a Superbill, and how to I get one?
It is an itemized receipt with the codes needed to submit a claim to your insurance provider.
You must request a Superbill from us, and can get it after your visit.
You are responsible for the entire process of submitting this claim to your insurance provider.
You might not be reimbursed the full “Self Pay” from your insurance provider. Latched Louisville has no control over this.
What your insurance may refund is dependent upon your coverage.
Some plans may require some amount of patient responsibility, e.g. a copay, coinsurance, or deductible.
